This service allows authorized users to request access to Form Fusion, a tool used for designing and managing document templates within Banner. Form Fusion enables the creation and customization of formatted reports, invoices, and other official documents. Users must complete a request form specifying their role and the level of access needed.
Who is Eligible?
Active Faculty or Staff eligible to use Form Fusion.
Prerequisites and Expectations
Each request is reviewed by the Enterprise Applications team for approval based on job responsibilities and university security policies. Once approved, access instructions will be provided.
