Who is Eligible?

Active faculty and staff who are eligible for PerfectForms.

How To Make your Form Inbox Ready

Step 1

  • Make an Input text Field and name it “ txtInstanceID “
  • Make an Input text Field and name it “ txtformname “ and enter the title of the name
  • Make an Input text Field and name it “check0 “add 0 in the Text box
  • Make an Input text Field and name it “check1 “add 1 in the Text box
  • Put it on your ATT page if you wish.

Step 2

Go to Form Open Make a set Field Box and map it to txtInstanceID

Step 3

Add a DBTest to your Routing Buttons
