Evisions Form Fusion Tax Template Installation

  • Revision date December 1st 2025
  • UTS EA Internal Documentation

Evisions Form Fusion tax template installation is essentially two parts: obtaining the Form Fusion tax templates from Evisions and then installing them in our TEST and PROD instances of Form Fusion.
Evisions Tax Templates include:

  • 1042S
  • 1095C
  • 1098T
  • 1099MISC
  • 1099NEC
  • 1099R
  • W2

Obtaining Form Fusion Tax Templates from Evisions

Go to https://evisions.com

  • Click on “Support & Community”

  • Click on "Product Support"

    Evisions1.JPG

  • Sign in with your Evisions account
    • This is local to Evisions - NOT Oakland SSO. You may need to reset your Evisions password

    Evisions3.JPG

  • Click on “FormFusion Taxes”

    • Evisions2.JPG

This is where Form Fusion tax templates are listed for download. Unfortunately there is no set date for when they are released so checking periodically or following the page is ideal. Evisions also does not seem to separate tax template releases from general discussion so a little digging may be required.

  • This section will be updated as new tax templates are released in 2025

Installing Form Fusion Tax Templates into Evisions

  • Log in to the Evisions eLauncher in either the TEST or PROD.
    • This will authenticate with LDAP/NetID credentials
